How to Get Started
at Our Practice
First and foremost, we'd like to welcome you to the Countryside Dental family! We have been proudly serving the community for more than a decade. Our team can't wait to welcome you to our Buffalo Grove office.
For your convenience, our New Patient forms are available for download in PDF format. Simply print and complete your forms, then bring them with you to your first appointment.
First Appointment Expectations
Your first visit to Countryside Dental will last for about one hour. During this time, we'll carefully assess your teeth and gums, take digital X-rays, and talk with you about your oral health history as well as any concerns you may have.
Following a thorough cleaning with one of our dental hygienists, Dr. Nagornaya will talk to you about the results of your exam and any recommendations regarding future treatment options. You'll have the opportunity to learn more about the cosmetic dentistry treatments we offer as well.
Please note all new patient are required to provide a credit to secure the appointment.
We require that you give our office 48 hours’ notice if you need to reschedule your appointment. A fee of $100.00 will be charged to you; this fee cannot be billed to your insurance company and will be your direct responsibility.

Your Child's First Dental Visit:
We happily welcome children to our Buffalo Grove dental office. Your son or daughter will receive a full exam, cleaning, fluoride treatment, and digital X-rays as needed. We will always spend extra time with our youngest patients, ensuring that they understand home hygiene and how diet affects oral health. We want your little one's smile to stay just as healthy as it is precious.
First Dental Visit Preparation
For your first visit to our office, please bring the following:
- Photo ID
- Your dental insurance card (if you will be using insurance)
- Your completed New Patient forms
New Patient Forms
Part of filling out patient forms is not just about learning when your last dental visit was. They also keep our dental office informed on your current health, whether it’s oral or systemic. For example, we ask about pre-meds, which can affect the initial visit. Please arrive 30 minutes early to fill up the appropriate paperwork. Once you’ve completed your new patient form, hand them to our front desk and we’ll get you seen as soon as possible for your exam, x-rays and hygienic cleaning.
Insurance Information
When you have dedicated dental insurance to cover your dental treatments, paying for your care becomes not just more affordable but more predictable as well. If your employer has a dental insurance benefit, it’s important that you utilize it to the fullest extent. Our office is happy to help you file claims on your behalf and answer any questions you may have about your plan or current coverage. For your convenience, we'll file your claims for you and do everything we can to maximize your available benefits.

Convenient Patient Financing
If you don’t have dental insurance, or even if you do but find you still need help paying for out-of-pocket expenses at the dentist’s office, patient financing can provide help filling in the gaps. CareCredit is a great financing option for many patients. Treatments costing $200 or more are eligible for payment plans across 12, 24, and 36 months.
